Instructions:

  • This is a 2-part assignment (Logic Model 1st)
    • PDSA Model due in a couple wks. but will be based off Logic Model
    • 2nd part due Sept 30th @ 12pm Central/Standard time
    • Template to use is attached
  • Develop a Logic Model & then develop a PDSA Model
  • Complete the Logic Model and use the rubric, exemplar, and resources provided to assist you. 
  • Take the logic model that you completed with your group. View the PowerPoint and change it to a PDSA. The PDSA should be more detailed than the logic model.

  • Incorporate the actions of your project in the Logic Model you use for Collaboration with the Project Site.
  • Apply DNP Project actions to the PDSA Model.
