Instructions:
- This is a 2-part assignment (Logic Model 1st)
- PDSA Model due in a couple wks. but will be based off Logic Model
- 2nd part due Sept 30th @ 12pm Central/Standard time
- Template to use is attached
- Develop a Logic Model & then develop a PDSA Model
- Complete the Logic Model and use the rubric, exemplar, and resources provided to assist you.
- Take the logic model that you completed with your group. View the PowerPoint and change it to a PDSA. The PDSA should be more detailed than the logic model.

Focus:
- Incorporate the actions of your project in the Logic Model you use for Collaboration with the Project Site.
- Apply DNP Project actions to the PDSA Model.
