Excellent coverage on the topic of child abuse. I have also wondered if a situation was abuse or if I was overreacting. Health care workers in lower income areas sometimes encounter patients with suboptimal living conditions. Parents that are very young or poor may have inadequate coping mechanisms thus, increasing risk for abuse or neglect. The cyclic process of poverty is an unfortunate situation needing to be addressed with social services that promote education and job placement.
However, until these reforms are enacted cases such as the Everman Texas and Cleveland Ohio child abuse instances will continue to occur. For example, Cleveland Ohio recently reported the death of a five year old female. The child was found by social services living in a home infested with bugs and covered with feces. The mother, reportedly had serious mental health issues which may have led to the children’s deplorable conditions. Unfortunately, one of her four children died. Although, the article does not say this, I wonder if the stigma of mental health and treatment was a factor in this horrific occurrence. I also wonder if someone noticed the children’s physical signs of neglect but due to reluctance of being wrong failed to report. I also wonder, if there were opportunities that I missed while working with children.
Another example of the need for child abuse education and reform includes the Everman child case. The child was reported missing in November, after family members became concerned that he was no longer present at gatherings. In addition a few weeks before the report the child was seen at the hospital with obvious signs of neglect and malnourishment. Months later investigators followed up and noticed the family moved out of the country. This child is presumed dead. As nurses, we have an obligation to report abuse however, due to fear of being wrong or overreacting I have found myself reluctant to call. I do not want to bombard the system with frivolous assumptions yet, I must advocate for the child. This gray area is a peril as sometimes the poor, young and minorities are treated differently by the healthcare and judicial system. These cycles of racisms reduce the probability of reporting as minorities and poor children make up a large portion of the foster system.
